Breakthrough Rehabilitation & Treatment Center is currently seeking a dedicated and skilled licensed Physical Therapist to join our team at our outpatient orthopedic facility in Lincroft, New Jersey. Work Environment

The work setting is exclusively onsite at our outpatient office in Lincroft, NJ, which has been designed to facilitate patient-centered care. Our operational hours are Monday through Friday, from 8:00 AM to 5:00 PM, allowing for a balanced work-life integration.

Key Responsibilities
  • Conduct thorough evaluations, assessing patients' functional abilities and disabilities, and developing tailored treatment plans based on comprehensive evaluations.
  • Implement individualized treatment protocols utilizing evidence-based practices, continually assessing and adjusting these protocols in response to patient progress and emerging needs.
  • Document all patient care activities accurately and in a timely manner, including initial evaluations, ongoing progress notes, and discharge summaries, ensuring compliance with relevant regulations.
  • Collaborate with multidisciplinary teams, engaging in consultations with physicians and other healthcare professionals to maximize patient outcomes.
  • Participate in organizational initiatives aimed at removing barriers to patient access and improving care delivery, utilizing data to drive performance enhancements.
  • Supervise and mentor junior staff or interns as needed, fostering a learning environment and promoting best practices in patient care.
  • Engage in continuous professional development by participating in training sessions, workshops, and team meetings to stay current with industry standards.
Patient Population / Client Focus

This position primarily caters to an outpatient population seeking orthopedic rehabilitation services. Patients may present with a variety of musculoskeletal conditions resulting from injury, surgery, or chronic conditions. As a Physical Therapist, you will be instrumental in guiding these individuals through their recovery journey, employing personalized care that considers both their physical limitations and personal goals.

Qualifications
  • A Doctorate of Physical Therapy (DPT) from an accredited institution.
  • Current state licensure as a Physical Therapist (PT) and CPR certification.
  • A minimum of two years of experience in an outpatient orthopedic setting.
Preferred Experience

While the core qualifications are essential, candidates with experience in advanced therapeutic modalities, manual therapy techniques, and a background in sports medicine or post-operative rehabilitation will be highly regarded. Familiarity with electronic medical records (EMR) systems and data tracking for performance outcomes is also beneficial.
