We are currently looking for a Certified Home Health Agency Registered Nurse (CHHA RN) to join our team as a Fee for Service employee, which provides flexible schedules and hours, patient location choices with a great work/life balance!

Certified Home Health Agency Registered Nurse (CHHA RN) Responsibilities:

  • Conducts home visits providing skilled clinical assessments, patient education, and direct nursing care to their patients in their own homes.
  • Coordinates skilled services within the patient’s interdisciplinary team as well as directs the patient’s paraprofessional services.

Certified Home Health Agency Registered Nurse (CHHA RN) Qualifications:

  • The ideal RN candidate is a dedicated, compassionate, and confident clinician with a current NYS RN license and strong organizational, communication, written, and customer service skills.
  • We expect our nursing candidate to be competent with Microsoft office and have a reliable mode of transportation to conduct home visits.
  • A BSN degree, prior CHHA experience, and a working knowledge of Home Care Home Base EMR are preferable.

Certified Home Health Agency Registered Nurse (CHHA RN) Duties include:

  • Assess patient and family to determine home care needs at the start of care and regular intervals.
  • Initiates Plan-of-Care and makes necessary changes as patient's needs evolve.
  • Evaluates and provides treatment to patients in their homes.
  • Instructs and advise patients and/or families in their care
  • Administers medications and treatments as prescribed by physician
  • Identifies and implements discharge planning needs as part of care development prior to discharge

Certified Home Health Agency Registered Nurse (CHHA RN) Requirements:

  • Valid & current New York State Registered Nurse License and Registration
  • Current Drivers License
  • Organized and Patient oriented
  • Able to work independently and have good oral and written skills
  • Minimum of 2 years clinical nursing experience
  • Certified Home Health Agency (CHHA) experience preferred
