Work Environment

As a remote position, the Licensed Professional Counselor role allows you to work from the convenience of your home, connecting with clients through telehealth solutions and digital platforms. Key Responsibilities

• Collaborate with primary care providers to identify, address, and manage mental health and psychosocial issues, acting as a vital member of the healthcare team.
• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, ensuring accurate assessments that inform treatment planning.
• Deliver brief, effective interventions for patients who require immediate mental health support, facilitating access to necessary resources.
• Consult with primary care providers to determine whether patients should continue care within a primary care setting or be referred to specialty mental health services.
• Initiate follow-ups with patients to monitor progress and adapt treatment plans based on evolving needs and circumstances.
• Develop comprehensive relapse prevention strategies and support patients in maintaining stable mental health functioning.
• Assess and identify vulnerable populations within your caseload, contributing to the development of strategies aimed at preventing deterioration of their condition.
• Coordinate health service delivery related to behavioral health by connecting patients with additional support providers, with their informed consent.
• Document patient progress and diagnostic information diligently in treatment records, ensuring adherence to healthcare regulations and standards.
• Advocate for patients’ needs and keep primary care providers informed regarding treatment plans and adjustments.
• Empower patients with self-management techniques and educational resources that promote participation in their treatment journey.
• Contribute ideas and consult on behavioral services, potentially influencing clinic management practices and program development.
• Assist patients in complying with medical treatments and provide strategies to mitigate any adverse effects from prescribed medications.
• Uphold the highest ethical standards in practice as defined by the Code of Ethical Conduct for Clinical Professionals.
• Assist with additional duties as assigned, demonstrating flexibility and commitment to team goals.

Patient Population / Client Focus

Our diverse patient population includes individuals from various age groups and backgrounds, many of whom face a range of mental health challenges influenced by socio-economic factors. You will engage with clients who require both immediate and ongoing support in managing mental health issues, utilizing evidence-based practices tailored for their unique circumstances. Qualifications

• A minimum of a Master's Degree in Clinical Counseling or a related field is required.
• Current and valid LPC (Licensed Professional Counselor) License is essential.
• Comprehensive knowledge of mental health assessments, treatment planning, and diagnostic frameworks including DSM-IV.
• Awareness of various therapeutic methodologies, including family therapy, group therapy, and behavioral therapy, to cater to diverse patient needs.
• Strong communication skills, with bilingual proficiency in English and Spanish preferred to enhance patient interactions.
• Adeptness in utilizing telehealth systems and maintaining strong digital communication with patients and healthcare teams.
• Robust understanding of community resources, enabling connected care and advocacy for patient support.
• Capacity to maintain personal and professional wellness to meet the demands of counseling roles effectively.

Preferred Experience

• Experience working within a Federally Qualified Health Center (FQHC) setting or similar healthcare environment is highly valued.
• Prior experience in a behavioral health role or providing integrated care alongside primary health services will be considered a substantial advantage.
• Skills in developing and implementing patient treatment plans based on thorough assessments and evidence-based practices.
• Familiarity with electronic health record (EHR) systems to ensure efficient and accurate documentation.

Compensation and Benefits

We offer a competitive salary range of USD 87,000 to 105,000 per year, commensurate with experience and qualifications.
