Position Summary

The Full-Time Infectious Diseases Nurse Practitioner (IDNP) will provide infectious disease services across partnered skilled nursing and long-term care facilities. This role focuses on evaluating, diagnosing, and managing infectious diseases, optimizing antimicrobial therapy, supporting infection prevention initiatives, and collaborating with facility staff and physicians to improve patient outcomes and reduce avoidable hospitalizations.

Key Responsibilities

  • Evaluate and manage patients with acute and chronic infectious diseases.
  • Develop and implement evidence-based treatment plans, including antimicrobial management.
  • Review laboratory results, cultures, and diagnostic testing to guide clinical decisions.
  • Conduct follow-up visits to monitor treatment response and adjust therapies as needed.
  • Collaborate with physicians, nursing staff, pharmacists, and the Vital Link clinical team.
  • Support antimicrobial stewardship and infection prevention initiatives.
  • Educate patients, families, and facility staff on infectious disease management.
  • Complete timely and accurate documentation in facility EMRs and Vital Link platforms.
  • Maintain compliance with state, federal, and facility regulations.
  • Travel between assigned facilities as needed.

Qualifications

  • Active Nurse Practitioner license (FNP, AGACNP, ACNP, or equivalent).
  • Medicare and Medicaid credentialed (or eligible).
  • Current BLS/CPR certification.
  • Experience in Infectious Diseases, Internal Medicine, Hospital Medicine, or Long-Term Care preferred.
  • Strong clinical assessment, communication, and documentation skills.
  • Familiarity with long-term care EMRs (PCC preferred).
  • Valid driver's license and willingness to travel.
