Job description

Job Objective: 

  • Responsible for evaluating and treating of Short Term Rehab and long term care residents with weekend rotation

Key Responsibilities: 

  • Evaluate and treat short term rehab and long term care residents.
  • Documentation in accordance with JCAHO, third party payers, and the Thompson Health System.
  • Program development, out come studies, and projects as required

Qualifications:

  • Valid NYS licensure to practice Physical Therapy
  • Degree in Physical Therapy from CAPTE approved program, DPT preferred
  • Prior experience in long term care and short term rehab setting preferred
  • Experience with EMR
  • Team player and self-motivated

Pay Rate: $47.00
