Oaks Integrated Health Centre is seeking a dedicated and experienced Licensed Professional Counselor (LPC) to join our team in Memphis, Tennessee. Work Environment

The work setting for this position is primarily remote, though you will occasionally engage with clients and colleagues through outpatient office visits. This flexibility allows you to maintain a positive work-life balance while contributing significantly to your community’s mental health needs. You will collaborate with various stakeholders, including supervisors, direct care staff, and family members, to enhance the support provided to clients.

Key Responsibilities
  • Conduct ongoing assessments to effectively evaluate and meet the emotional and psychological needs of patients.
  • Facilitate communication among supervisors, families, and other relevant parties regarding clients’ progress and treatment plans.
  • Provide a tentative schedule to supervisors for appointments, home visits, and observations, ensuring a coordinated approach to patient care.
  • Coordinate home visits to fulfill requirements for in-home observations, data analysis, and chart reviews.
  • Ensure that the implementation of the therapeutic program plan is effective and compliant with established protocols.
  • Participate actively in Interdisciplinary Team Meetings to ensure comprehensive client care.
  • Prepare and complete monthly and quarterly progress notes to document client developments.
  • Organize training sessions to certify staff members in the implementation of program plans and required documentation.
  • Provide ongoing follow-up interviews and home visits to support clients continuously.
  • Maintain accurate records to ensure information is accessible to authorized parties and aids in continuity of care.
  • Act as a liaison between clients and the public mental health community to strengthen support structures and development goals.
  • Assist in developing training materials and procedures to enhance program effectiveness.
Patient Population / Client Focus

In this role, you will work with a diverse range of clients, including both adults and youth, many of whom may be dealing with issues such as anxiety, depression, trauma, substance misuse, and interpersonal conflicts. The focus will be on delivering tailored therapeutic interventions that cater to the unique circumstances and needs of each individual, thus fostering a supportive and nurturing environment.

Qualifications
  • A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field.
  • Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Tennessee.
  • A minimum of two years of clinical counseling experience, demonstrating a deep understanding of mental health issues.
  • Strong knowledge of legal and ethical standards in mental health practice.
  • Exceptional interpersonal and communication skills, with a proven ability to build rapport with clients from diverse backgrounds.
