Job description

We are seeking a licensed Clinical Social Worker (LCSW) to support inpatient discharge planning and psychosocial services within an acute care hospital.

Position Overview

The LCSW will assess patients’ psychosocial needs, coordinate discharge planning, and provide crisis intervention and counseling services.

Responsibilities
  • Conduct psychosocial assessments

  • Develop safe and effective discharge plans

  • Coordinate with case management, nursing, and providers
