Registered Nurse - Home Health
Full Time / Competitive Pay
Registered Nurse Job Summary:
As a Registered Nurse - Home Health, you will provide skilled nursing care to patients in their homes, managing a wide range of medical conditions and promoting health and wellness. You will work closely with a multidisciplinary team to ensure that patients receive the highest standard of care, tailored to their individual needs.
Registered Nurse Job Requirement
  • Graduate of an accredited program for nursing education, BSN preferred.
  • Current licensure to practice professional nursing in the State of Ohio.
  • Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.
  • Valid Ohio's Driver License and maintain motor vehicle insurance
Registered Nurse Key Responsibilities
  • Conducts an initial assessment of the patient and family to identify home care needs, including a thorough physical assessment and history of current and past illnesses.
  • Regularly reassesses the patient's nursing needs to ensure care is adjusted as necessary.
  • Develops and implements an individualized care plan tailored to each patient's needs, making revisions as required.
  • Completes patient charting by the start of each workday or within 72 hours of the visit.
  • Ensures OASIS charting is completed within three (3) days of the visit.
  • Utilizes health assessment data to establish accurate nursing diagnoses.
  • Creates a care plan that sets goals based on nursing diagnoses and integrates therapeutic, preventive, and rehabilitative nursing actions, involving both the patient and family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures, administering medications and treatments as prescribed by the physician.
  • Provides counseling to the patient and family to support their nursing and related needs.
