Job description

Nurse Case Manager

Schedule: Monday - Friday 8:30am - 5:15pm

Under the general guidance of the Director of Care Management, the Nurse Case Manager will provide direct patient care through implementation of individual patient care plans developed in collaboration with the patient's primary care provider. This individual will work directly with the provider for continuity of care to fulfill the individual patient care plan. The Nurse Case Manager will promote self-care management and assist with access to care. Functions through direct face to face patient contact, telephone follow-up and education based on care management guidelines in a sensitive and confidential manner. Maintains satisfactory relations interdepartmentally and participates in phases of center wide health education.

Responsibilities:

The Nurse Case Manager role involves the use of critical thinking skills and clinical expertise to provide ongoing case and care management to an assigned panel of patients. This will include:

  • Managing the ongoing care management for an assigned population case load (Population Health Management)
  • Performing Comprehensive Risk Assessments and developing individualized Patient Care Plans for all assigned cases
  • Managing and coordinating patient access to care, including overseeing the efforts of collaborating Care Coordinators, Referral staff and others
  • Addressing all relevant Transitions of Care management when patients obtain hospital-based care
  • Collaborative participation in the Patient Care Teams
  • Population Health Management
  • Development & Management of Patient Care Plans includes management and coordination of care
  • Transitions of Care and Self Management

Qualifications:

  • Associate degree in Nursing from an accredited college or university is minimally required. Bachelor's degree in Nursing from an accredited college or university is required to facilitate complex disease care management and advanced patient needs.
  • Licensed Practical Nurse (LPN) or Registered Nurse (R.N.) with a valid license in the state of Illinois
  • Current CPR Certification
  • A minimum of two (2) – three (3) years' experience as a qualified Case Manager preferred.
  • Significant demonstrable experience of working in a community-based environment.
  • Significant demonstrable experience of working with highly complex packages of care in a health and/or social care environment.
  • Bilingual ability preferred; may or may not be required based on individual department needs.
