Job Title: Inpatient Care Manager (RN/Case Manager)About the Facility

Position Overview

We are seeking an experienced Inpatient Care Manager to support care coordination, discharge planning, and resource management across multiple inpatient units. This role is ideal for someone who thrives in a collaborative environment and is passionate about improving outcomes for complex and underserved patient populations.

Unit Overview

  • Hospital-wide role (unit sizes vary)

  • Assignment to a primary unit with potential to float across inpatient units

  • Works closely with interdisciplinary teams to ensure safe and timely patient transitions

Patient Population

  • Primarily adult and geriatric patients

  • Complex cases including:

    • Unhoused and low-income populations

    • Patients with limited access to resources

  • May support units such as:

    • Med/Surg

    • PCU

    • LDRP

    • (ICU coverage possible but less common)

Caseload

  • Typically 14-20 patients

Required Qualifications

  • Minimum 3 years of acute care case management experience

  • First-time travelers: Accepted

Education & Certifications:

  • ASN or higher required

  • No specific certifications required

Required Skills

  • Strong care coordination and discharge planning experience

  • Ability to:

    • Collaborate effectively with physicians and multidisciplinary teams

    • Navigate insurance plans and authorization processes

    • Identify and coordinate community resources

  • Excellent:

    • Assessment and critical thinking skills

    • Communication and interpersonal skills

  • Must be a team player with a proactive approach

Floating

  • Assigned to a primary unit but may float across inpatient units as needed

Orientation

  • New Employee Orientation (NEO)

  • 3 shifts of department orientation (with possible extension if needed)

Scheduling

  • Shift: Monday-Friday, 8:00 AM - 4:30 PM

    • Occasional late days as needed

  • Weekend Rotation: Every 5th weekend

    • If working weekend, receives following Monday & Friday off

  • On-Call: Not required

  • Holiday Requirement: Approximately 50% (up to 2 holidays per contract)

  • Schedule Type: Set schedule with weekend rotation
