Position Overview

We are seeking a dedicated and fully licensed Licensed Clinical Social Worker (LCSW) to join our compassionate team at Portia Clever Health Association Service. As part of our remote mental health clinic, you will deliver high-quality psychotherapeutic services to patients at our Hospice Center in Rochester, New York. This full-time role offers the unique opportunity to work with terminally ill patients, their families, and caregivers, supporting them in navigating complex emotional and psychological challenges associated with end-of-life care.

Key Responsibilities

As a Licensed Clinical Social Worker, your primary obligation will be to provide comprehensive psychotherapy services tailored to individuals at the Hospice Center. Your specific responsibilities will include:

  • Conducting thorough clinical assessments to understand patients' mental health needs and develop individualized treatment plans based on established best practices.
  • Implementing a variety of evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), customized to meet the unique needs and preferences of terminally ill patients and their families.
  • Maintaining precise clinical documentation that complies with HIPAA standards, ensuring all records are accurate and accessible while prioritizing patient confidentiality.
  • Collaborating closely with an interdisciplinary team, including psychiatrists, psychologists, nurses, and other mental health professionals, to coordinate care and enhance treatment effectiveness.
  • Engaging in regular case consultations and peer review meetings, thereby improving the quality of care through shared insights and best practices.
  • Offering education and support to patients and their families about mental health issues, coping strategies, and available community resources.
  • Keeping abreast of the latest research developments in mental health and integrating new knowledge into clinical practices to improve patient outcomes.
Patient Population / Client Focus

This role primarily focuses on providing support to patients receiving hospice care, which may include individuals coping with terminal illnesses, their families, and caregivers. You will approach each case with empathy and respect, addressing not only mental health needs but also the emotional and psychological complexities accompanying end-of-life scenarios.

Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • A valid license to practice as a Licensed Clinical Social Worker (LCSW) in New York State.
  • A minimum of 2 years of clinical experience, specifically in psychotherapy, with a demonstrated ability to work competently in hospice or healthcare settings.
  • Expertise in solution-focused psychotherapy techniques and the ability to utilize various therapeutic modalities effectively.
  • Exceptional communication and interpersonal skills, facilitating positive interactions with patients and team members.
  • Strong analytical and problem-solving skills, enabling you to make independent clinical judgments.
  • Proficiency in utilizing electronic health records and telehealth platforms to manage patient information efficiently.
Preferred Experience
  • Experience in hospice care or working with patients facing chronic or terminal illnesses is highly desirable.
  • Familiarity with grief counseling and bereavement support would be advantageous.
  • A commitment to ongoing professional development and education in mental health practices.
