About the Role:

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Health team in the Visalia, CA area. As a Registered Nurse in Home Health, you will be responsible for providing skilled nursing care to patients in their homes, ensuring their safety, comfort, and well-being. You will work closely with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for each patient, and provide education and support to their families. Your ultimate goal will be to help patients achieve their highest level of independence and quality of life.

Minimum Qualifications:

  • Active and unrestricted Registered Nurse license in the state of California
  • Minimum of 1 year of recent experience in Home Health or related field
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Responsibilities:

  • Conduct initial and ongoing assessments of patients' health status and needs
  • Develop and implement individualized care plans based on patients' conditions and goals
