Licensed Practical Nurse (LPN) – Provider Support & Clinical Coordinator
MFM Health is seeking a compassionate, highly organized, and proactive full-time Licensed Practical Nurse (LPN) to join our growing primary care team in Danvers, MA This is an excellent opportunity for an LPN who enjoys working closely with providers, thrives in a fast-paced environment, and wants to play a vital role in improving patient care and provider efficiency.
As a Provider Support & Clinical Coordinator, you will become an essential partner to our physicians and advanced practice providers by coordinating clinical workflows, managing patient care tasks, and ensuring seamless communication between patients, providers, and the healthcare team.

Key Responsibilities
Provider Support & Clinical Workflow
  • Partner closely with assigned physicians and advanced practice providers to support efficient daily clinical operations.
  • Function as a clinical scribe during patient visits, accurately documenting encounters within the electronic medical record (EMR).
  • Anticipate provider needs to improve workflow efficiency and maintain continuity of patient care.
  • Review and prepare patient charts prior to appointments, ensuring records are complete and identifying outstanding clinical needs.
  • Scrub provider schedules to identify care gaps, overdue preventive services, referrals, testing, and follow-up requirements.
  • Assist with provider inbox management by prioritizing and completing clinical tasks in a timely manner.
  • Support medication management, including refill requests, prior authorizations, prescription questions, and medication-related follow-up.
  • Review laboratory, imaging, and specialty reports for appropriate routing and provider action.
  • Maintain accuracy while managing multiple priorities in a high-volume clinical environment.
Clinical Coordination
  • Coordinate referrals, specialty appointments, diagnostic testing, and outside medical records.
  • Facilitate communication between patients, providers, specialists, pharmacies, and ancillary healthcare organizations.
  • Assist with reviewing, organizing, indexing, and labeling incoming clinical documentation within the EMR.
  • Ensure timely completion and routing of provider tasks, patient messages, test results, and clinical documentation.
  • Support continuity of care by helping patients navigate recommended follow-up care and treatment plans.
Team Support & Collaboration
  • Provide clinical support and coverage for Medical Assistants and nursing staff as needed.
  • Collaborate effectively with providers, nurses, medical assistants, care management teams, and administrative staff.
  • Adapt to changing patient volumes, provider schedules, and evolving clinical priorities.
  • Maintain professionalism while working with diverse communication styles and interdisciplinary teams.
  • Foster a positive, collaborative, patient-centered work environment focused on delivering exceptional care.

Qualifications
  • Current Licensed Practical Nurse (LPN) license in the Commonwealth of Massachusetts.
  • Previous experience in primary care, family medicine, or ambulatory care preferred.
  • Experience with electronic medical records; Athenahealth experience preferred.
  • Strong understanding of clinical workflows, care coordination, and provider support.
  • Excellent organizational, multitasking, and time management skills.
  • Strong written, verbal, and interpersonal communication skills.
  • Ability to work independently while collaborating effectively with providers and multidisciplinary teams.
  • Demonstrated critical thinking, attention to detail, and adaptability in a fast-paced clinical environment.

Pay Range: $34 - $40 per hour; Commensurate with experience
