Provider's Role with the Company:
  • Responsible for providing home nursing care through all phases of care
  • Provide treatment to patients per protocols, utilize OASIS assessment and documentation.
  • Manage and administer prescription medicines as needed
  • Perform on-going periodic system assessment and data collection in a systematic manner
  • Educates and instructs the patient, patient's family, and other staff as required.
  • 1 weekend on-call

Qualifications
Background Profile:
  • Registered Nurse/RN
  • License and Board Certification in the state of Virginia or pending
  • Open to new graduates with a great clinical background
  • 1 A year or more home health experience preferred
  • Team-oriented & outstanding communication skills
