Primary Care Physician Scope -Quals Catawba Service Unit

Description

Physician shall act as the attending Physician providing and managing patient care at the Catawba Service Unit The clinic operates in Rock Hill, SC. The services shall include but not limited to providing medical care for urgent, preventative, chronic, and progressive care to pediatric, geriatric, adolescent and adult patients. Treat the fill range of family practice cases and perform most diagnostic and treatment procedures without further direction. The physician will instruct patients on specific treatments as well as in preventative care. They will observe and abide by the medical staff bylaws, service unit policies and procedures and all applicable federal and state rules and regulations. The physician must complete forms directly relates to patient care, which must be completed by a physician and issue all patient referrals, as needed. Physician will clinically supervise two Physician Assistants and oversee the laboratory. The physician must have an active South Carolina Medical License. Physician must be able to be CLIA certified or gain the requirements within one month of starting employment. Physician shall be work 4 8 hour days on Monday, Tuesday, Thursday and Friday, and clinic session shall be during normal clinic workings hours of 8:00am to 4:30 pm, unless approved in advance by CEO and COR.. At the request of the Chief Executive Officer or there alternate, Physician shall attend and complete all mandatory classes and trainings. The Catawba Service Unit will not compensate Physician for any services provided outside of those specifically identified in scope of services without prior authorization, evidences only be written change order or addendum to this agreement executed by Catawba Service Unit. Physician shall be working at the Catawba Service Unit located at 2893 Sturgis Road, Rock Hill, SC 29730 or other locations mutually agreed upon by Physician and Catawba Service Unit Chief Executive Officer. Physician agrees to maintain as required by law or the Catawba Service Unit accurate medical records, which sufficiently and properly reflect the services performed by the physician. Such medical records shall be and remain the property of the Catawba Service Unit and shall be subject to retention by the Catawba Service unit in accordance with all federal, state and local laws applicable to medical records. Physician shall also be responsible for handling all notifications in medical records in timely manner as set by Catawba Service Unit policies. Insurance: For all of the services set forth herein and as hereinafter amended, Physician shall maintain or cause to be maintained, in full force and effect during the term of this agreement, at his or her own expense, malpractice insurance, and keep on file with the Catawba Service Unit a current certificate of coverage. Notwithstanding anything to the contrary in this agreement, physician irrevocable waives all claims against the Catawba Service Unit for all losses, damages, claims or expenses resulting from risks commercially insurable under this insurance. In performing services under this contract, physician shall operate as, and have the status, of independent contractor and shall not at as an agent, or be an agent of the Catawba Service Unit. In accordance with such status as independent contractor, physician covenants and agrees that it will not hold herself or himself out as, nor claim to be an officer or employee of the Catawba Service Unit, or of any department, agency or unit thereof by reason hereof, and that he or she will not, by reason hereof, make any claim, demand or application to or for any right or privilege to an officer or employee of the Catawba Service Unit including, but not limited to Worker's Compensation coverage, health coverage, unemployment insurance benefits, Social Security coverage or employee retirement membership or credit.
