OVERVIEW

Assists the Manager in the operation of an Intermediate Care Facilities (ICF) providing residential care and programs on a 24/7 basis in service to the medical, physical, psychological and social needs and personal goals for intellectually and developmentally disabled adults having more complex medical or behavioral needs. Work is performed under regular supervision and is guided by well-established and specific agency policies and procedures, quality assurance, state and federal licensure and accreditation regulations and guidelines and ethical standards related to client interactions.

DUTIES AND RESPONSIBILITIES

• Assists in the training, scheduling, supervising and evaluating residential staff.
• Assists in monitoring programs and facilities for compliance to state and local licensure, Virginia Department of Health, and other funding source requirements.
• Provides direct care, physical assistance and training to residents in support of daily living including but not limited to personal hygiene, transportation, medication administration, laundry, meal preparation, serving, feeding and cleanup, shopping, community activities, and appointments.
• Observes residents, monitors needs and progress toward goals and enhances service plans in collabora­tion with staff, the residents, their families, and other service providers.
• Records relevant progress notes; completes and maintains required charts, forms and reports.
• Participates as a member of the Personal Support Team for Annual Person Centered Plan meeting and semi-Annual reviews.
• Transports residents to community based supports, services and activities
• Assists the medical staff in the administration of medication, and preparation of related records.
• Assists residents with personal financial transactions and record keeping.
• Communicates with family members, and other appropriate persons.
• Provides day and/or overnight supervision to residents; responding to needs as necessary.
• Functions as food manager, in conjunction with the Registered Dietician and shops for groceries.
• Conducts fire drills, inspects smoke detectors and fire extinguishers according to policy, and maintains appropriate records.
• Provides First Aid and CPR as needed.
• Provides interventions with behavioral issues and responds to medical emergencies.
• Prepares functional assessments of resident's skills, preferences, needs, strengths, and desires.
• Assists in residential placements.
• Supervise facility visitation.
• Liaises with resident families, legally authorized representatives, legal guardians, Interdisciplinary Team members, contracted physicians, specialists, day support providers, and medical professionals concerning resident care and affairs.
• Helps coordinate the maintenance of ICF facilities and vehicles.
• Assists in the development, implementation, monitoring, evaluating, and revising ICF policies and procedures.
• Monitors and advocates for the human rights of residents.
• Monitors and documents the condition, maintenance and repair of facility medical/adaptive equipment
• Assists with resident medical appointments and follow-up.
• Inputs service and other documentation to the electronic records system.
• Does related work as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES

Thorough knowledge of the theories, principles, practices and techniques related to the care and treatment of individuals with intellectual disabilities; thorough knowledge of the common disabilities of the individual population; thorough knowledge of human rights of residents; thorough knowledge of services and programs available to the residents within the agency and by external service providers; thorough knowledge of liability issues; considerable knowledge of service plan development; considerable knowledge of case management principles and practices.

Considerable knowledge of the methods and use of electronic health records; considerable knowledge of the maintenance, security and safety practices and procedures related to the operation of a residential facility; considerable knowledge of state licensure requirements; ability to estab­lish effective working relationships with a variety of others; ability to communicate effectively orally and in writing; ability to operate a vehicle; ability to collect and analyze data from a variety of sources including electronically and prepare reports; ability to plan, train and supervise the work of others; ability to function calmly in stressful situa­tions; ability to work with individuals and family members with challenging behaviors; ability to interact positively and respectfully with clients and their families; ability to conduct effective interviews, observe, assess and record individual behaviors and functioning and develop treatment and service plans.

TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE

Bachelor's Degree in the Behavioral Sciences, and considerable experience in residential treatment programs for intellectually disabled/developmentally disabled adults.

LICENSURE AND CERTIFICATIONS

Valid Virginia driver's license. Certification in First Aid/CPR. Training in Therapeutic Options (THEROPS) and Medication Management.

Must possess all qualifications and skills required by federal and state regulations to bill Medicaid for the services provided. See Medicaid Billing Addendum.

ENVIRONMENTAL AND PHYSICAL EXPOSURE

Work involves considerable exposure to disagreeable environmental conditions such as the requirements to interact with clients having unsanitary behaviors, poor hygiene and contagious illness and are hostile, aggressive, or potentially violent; drive a vehicle and/or transport clients in inclement, if not severe weather; regularly physically assisting and/or lifting clients in the course of care and lifting moderate weight objects in connection with the operation of the facility; is frequently interrupted during off-hours with stressful or emergent calls.

RELATIONSHIPS

Relationships require excellent communication and interpersonal skills to represent the program within the department and to community and state partners, organizations and agencies or other stakeholders concerning program initiatives and services and the ability to use appropriate techniques to counsel and manage the behaviors of program participants.
