As a Home Health Wound Care Registered Nurse, you will serve as the clinical wound care expert for our home health team. In this role, you will provide clinical consultation and support to field clinicians by reviewing wound care documentation, evaluating complex cases, and collaborating with physicians and interdisciplinary teams to develop evidence-based treatment recommendations. Your expertise will help improve patient outcomes, promote regulatory compliance, and ensure the delivery of high-quality wound care across the organization.

As the Home Health Wound Care Registered Nurse, you will play a key role in supporting clinicians who provide direct patient care in the home. Rather than managing a daily patient caseload, you will use your clinical expertise to review wound care documentation, provide recommendations on treatment plans, and serve as a wound care resource for the clinical team. You will work closely with physicians, nurses, and interdisciplinary partners to promote evidence-based wound care practices, optimize resource utilization, and support quality improvement initiatives that enhance patient outcomes.

Responsibilities

  • Serve as the clinical wound care resource for home health field clinicians by providing consultation and guidance on complex wound cases.

  • Review clinical documentation and wound assessments submitted by field staff to ensure appropriate treatment planning, regulatory compliance, and adherence to agency guidelines.

  • Collaborate with physicians and interdisciplinary team members to develop individualized, evidence-based wound care treatment plans.

  • Provide clinical recommendations for the management of acute and chronic wounds, including pressure injuries, diabetic foot ulcers, venous and arterial ulcers, burns, surgical wounds, skin tears, and other complex skin conditions.

  • Develop, implement, and promote standardized wound care protocols and evidence-based best practices across the organization.

  • Educate and support clinicians on wound care techniques, dressing selection, infection prevention, and wound management strategies.

  • Provide consultation and support for ostomy management, including colostomy and ileostomy care, while assisting clinicians with ostomy-related complications.

  • Support clinicians in the prevention and management of skin integrity issues related to urinary and fecal incontinence.

  • Monitor wound healing progress and evaluate treatment effectiveness, recommending modifications to the plan of care when appropriate.

  • Participate in utilization management by promoting appropriate resource utilization while maintaining high-quality patient outcomes.

  • Conduct prospective, concurrent, and retrospective clinical reviews to ensure compliance with agency policies, regulatory standards, and evidence-based practices.

  • Analyze wound care outcomes and identify opportunities for quality improvement and enhanced clinical performance.

  • Collaborate with the Director of Nursing, Case Managers, physicians, nurse practitioners, therapists, dietitians, and other interdisciplinary team members to support comprehensive patient care.

Qualifications

  • Active Registered Nurse (RN) license in Florida.

  • Previous home health nursing experience required.

  • Previous wound care experience required.

  • Wound Care Certification (WCC, WOCN, CWCN, or equivalent) preferred.

  • Strong knowledge of home health regulations, Medicare guidelines, OASIS, and clinical documentation requirements.

  • Demonstrated knowledge of evidence-based wound care practices, skin integrity management, and ostomy care.

  • Strong clinical assessment, critical thinking, communication, and collaboration skills.

  • Ability to review clinical documentation, provide recommendations, and effectively support field clinicians in a collaborative environment.

  • Proficiency with electronic medical records (EMR) and clinical documentation systems.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
