The office operates Monday through Friday from 8:00 AM to 5:00 PM, with occasional evening shifts required to accommodate patient needs. This role entails engaging patients via telehealth platforms, ensuring that the therapeutic environment remains conducive and supportive despite the distance.

• Conduct thorough mental health assessments that consider how medical conditions, such as chronic pain or autoimmune disorders, impact patients' psychological well-being.
• Develop and implement effective integrated treatment plans, focusing on both psychiatric and medical considerations to facilitate comprehensive care.
• Provide individual and group therapy sessions using evidence-based practices tailored to the unique circumstances and needs of each patient.
• Collaborate with an interdisciplinary team of medical providers to ensure that treatment is coordinated and outcomes are optimized.
• Deliver crisis intervention services as necessary, empowering patients during mental health emergencies.
• Educate and empower patients regarding the interconnections between their mental health and physical conditions, enhancing their ability to manage their well-being.
• Maintain accurate, confidential, and timely documentation in compliance with legal and ethical guidelines, utilizing electronic health record (EHR) systems.
• Actively engage in team meetings, case conferences, and care planning sessions to foster collaboration and continuous improvement.
• Stay informed on research and advancements in integrated care to provide the highest quality therapeutic interventions.

This role primarily involves working with individuals facing a variety of mental health challenges alongside chronic medical conditions. Patients may include those dealing with substance use disorders, complex pain management issues, and other co-occurring medical and psychiatric conditions. Your expertise will be vital in tailoring interventions that consider these multifaceted issues, ensuring an empathetic and comprehensive approach to treatment.

• A Master’s or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Must possess a current and valid licensure as an LCSW, LCPC, or LMFT in the state.
• Solid proficiency in addressing the mental health needs of individuals with co-occurring medical conditions, ideally with a background in integrated care settings.
• Demonstrated ability to collaborate effectively with medical professionals, fostering a team-oriented and coordinated approach to treatment.
• Strong interpersonal and communication skills to facilitate the build of trusting therapeutic relationships with patients.
• Commitment to maintaining professional boundaries alongside adherence to ethical guidelines and patient confidentiality.
• Experience with electronic health record systems and familiarity with teletherapy tools and platforms.
• A collaborative mindset, capable of functioning as part of a multidisciplinary team.

• At least two years of direct counseling experience in a clinical setting is highly desired.
• Experience working specifically in an outpatient environment that addresses integrated care models.
• Prior involvement in interdisciplinary teams, showcasing a commitment to holistic patient care.
