Clinical Coordinator - Hospice - Hudson Valley-Days

Working at NewYork-Presbyterian Hudson Valley Hospital, one of the newest members of the NewYork-Presbyterian network, you'll impact the lives of thousands while continuously growing both professionally and personally.

This position requires an RN experienced and knowledgeable in hospice care. Also, an understanding of the Medicare hospice benefit, hospice diagnoses, and end-of-life disease processes is essential. You'll be a frontline contact for patients, families, physicians, and hospitals for hospice referrals. Responsible for coordinating patient services and communicating with colleagues in the field. This is an opportunity to join a strong, compassionate, growing team of professionals committed to quality end-of-life care in patients' homes, skilled nursing facilities, and hospitals.

This is a full-time day shift opportunity working 5 days a week, working 10 am -6 pm or 11 am to 7 pm. Will be required to work some weekends and holidays. The location for this role will be Hudson Valley. Responsibilities include, but are not limited to, admitting and coordinating care for inpatient hospice patients, rounding with the hospital interprofessional medical teams, facilitating hospital discharges out of the hospital to the Hospice community program, and performing other duties as required.This position is located at Jansen Hospice and Palliative Care, a Joint Commission Accredited Certified Hospice Agency.

Preferred Criteria

5-7 years of experience

Ability to be flexible, apply critical thinking skills, and function independently.

Capability to work as a strong, committed member of our hospice interdisciplinary team of professionals.

Must have two years of experience in a hospice or home care agency.

Required Criteria

Bachelor's Degree in Nursing

Current New York State RN license

BCLS certification (American Heart Association)

Must have a Driver's License and a reliable car

Excellent communication skills and computer proficiency required.
