General Description
The Public Health Assistant is a valuable member of our Prevention team who performs clerical support for IBCCP Program as assigned. Responsible for determining program eligibility, scheduling appointments, data entry, translation for Spanish-speaking clients, filing, telephone contacts, correspondence duties, and community outreach.
Scheduled Hours: 19 hours per week
  • 2-3 days a week
  • Flexible work shifts, generally 9:00AM - 2:00PM
Essential Functions
  • Determines eligibility for services by screening clients according to program requirements and guidelines.
  • Performs general office tasks.
  • Accurately performs data entry of program-related data and information in the Cornerstone and AS400 computer system.
  • Assists in educating the public, potential clients, and families, in community outreach activities.
  • Schedules clients for IBCCP Program appointments, follow-up appointments, and tracks results and recommendations under direct supervision.
  • Performs verbal language interpretation and written language translation as needed.
  • Develops and maintains a basic knowledge of services available at the Lake County Health Department and Community Health Center (LCHD/CHC).
  • Develops and maintains a basic awareness of the Chronic Care Model Processes in order to reduce health disparities within our communities.
  • Develops and maintains a working knowledge of individual Emergency Preparedness roles.
  • Demonstrates adherence to National Patient Safety Goals.
  • Participates in quality improvement activities to assess, monitor and improve program services.
  • Participates in mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development.
  • Reports all unusual occurrences that may involve the safety and well being of the clients, patients and/or co-workers, directly to the supervisor and/or Safety Officer.
  • Assists and encourages clients/patients in identifying self-management goals.
  • Performs duties in a prioritized, organized and orderly manner to maximize clinic efficiency and productivity.
  • Maintains orderliness and cleanliness of clinic and work area.
Knowledge Skills Abilities and Education Required
  • Requires a High School Diploma or GED.
  • Requires one-year general office experience.
  • Data entry skills.
  • Basic knowledge of computers.
  • Assist in bilingual translation as appropriate.
  • Must possess sufficient physical stamina to allow for lifting, pushing, and pulling of clients/patients or other objects, regularly exerting up to 35 pounds.
  • Ability to work on weekends holidays and overtime if necessary.
  • Ability to maintain and keep records.
