About the Organization and the Job:

Heart of Hope Asian American Hospice Care is a non-profit startup Hospice licensed by California state since 2016. We are looking for a per-diem Home Health Aide (HHA) to join our growing team in San Jose, California. It offers a unique opportunity to work with a team that is passionate in providing end-of-life care to the under-served community in the Silicon Valley Area.
High-level Job Description

Under the supervision of the registered nurse, the HHA provides personal care and related non-professional household services necessary to meet the hygiene, ADL, and comfort needs of the homebound patient in alignment. The HHA receives functional guidance and supervision from the interdisciplinary team. The HHA will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
Pay Rate and Benefits

Pay rate depends on relevant qualification and experience.
Hourly rate: $22-$27
Visit rate: $50-$55

Qualifications/Requirements

  • Required Licensure and Certification
  • Must be a Certified Home Health Aide in the State of California
  • Basic Life Support for Healthcare Providers (BLS) CPR certification
  • Automobile insurance
  • Driver License, valid and in the State of California
  • Compliant with Covid-19 healthcare personnel requirement in all City, County, State and Federal level
Experience and Skills

  • Minimum one year experience performing duties as an aide in a home care setting or other health care related facility required
  • Direct patient care experience within the past three years required
  • Working knowledge of medical terminology and health care practices is required as well as understanding of universal precautions
  • Ability to read and follow written instructions and document care given
  • Tolerance for body secretions, strong odors, potential exposure to disease, and varied environmental conditions
  • Able to cope with emotional stress tolerant of different lifestyles, cultures, beliefs, and values
  • Knowledge of and commitment to hospice philosophy of care
  • Bilingual Chinese/English Speaking welcomed but not required
