Overview

Salary Range $35.50 - $53.79 Hourly Position Type Per Diem/Pool Education Level 4 Year Degree Category Allied Health

Description

Coordinates all phases of nutritional care including screening, assessing, monitoring, educating and documenting. Responsible for practicing and ensuring adherence to SJH Corporate Compliance Program. Provides age and developmentally appropriate clinical care and optimal nutrition information to all populations. Essential Functions 1. Clinical Care Uses the Nutrition Care Process to accurately and consistently assess patients and formulate a plan of care. Functions as a member of the clinical interdisciplinary team. Communicates any recommendations or issues with clinical staff and documents all interventions in the medical record. Communicates diet restrictions/special diets to patients and room service staff. Responsible for sharing weekend call duty with other dietitian(s) for all patient care areas, including assessment of all nursing screens and completion of all weekend consults. 2. Critical Care and Specialty Clinic Support Formulates specialized diets including TF and TPN. Works with physicians and pharmacists to assure TF and TPN provide optimal nutrition support. Completes high risk assessments in a timely manner. Provides consults to the Oncology department and keeps oncology education information updated. 3. LTC Competence Be able to screen, monitor, assess and care plan for LTC residents, and skilled/IRF patients according to standards of care and regulatory agencies. Participate in care conferences, monthly chart checks and weekly reviews of nutritionally at risk residents/patients. Updates diet sheets and assures that resident information and menus are accurate and appropriate. Provide diet consultations to skilled and LTC population. 4. Other duties as assigned Participation on committees, attending meetings as requested, PI projects, assuring that Nutrition Services meet all State and DNV survey standards, updating policies, providing education for the diet aides in special diets and customer service, in-services and nutrition education or wellness programs. 5. Skill Development Maintains, develops and updates skills to implement ideal care and service. Keeps licenses and certifications current.

Minimum Education Required: BS in Nutrition Preferred: MS in Nutrition Minimum Work Experience Required: Completion of clinical experience as required by the Academy of Nutrition and Dietetics Preferred: Two years experience in Clinical Dietetics, Experience with a computerized medical record, Experience in a LTC setting, trained as a Nutrition Focused Physical Exam provider. Required Licenses: Registered Dietitian with the Academy of Nutrition and Dietetics and Licensed Dietitian with the State of Wyoming Functional Demands Working Conditions: Walking, standing and sitting at a computer. Hospital environment, stressful situations. Physical Requirements: Must be able to lift 50 lbs. Ability to lift, push, pull, and move patients, equipment
