Home Health LPN
Location:
This Home Health RN position serves patients throughout Chicago's West Side, Near West Side, and Near South Side neighborhoods. The territory includes communities such as North Lawndale, Little Village, East and West Garfield Park, Humboldt Park, Hermosa, Avondale, Logan Square, Pilsen, the Near West Side, University Village, Bridgeport, Chinatown, and Bronzeville. Care is provided within the city of Chicago, with patient assignments geographically aligned to support efficient travel and continuity of care.
Schedule: Full-time, 40 hours, Days, M-F, rotating weekends, plus 1 summer/winter holiday
Sign-On Bonus Eligible
This position is eligible for a sign-on bonus up to $5,000 for external candidates. Payout is based on employment in the exact role at the time of payout. Internal team members are not eligible.
How You Will Make a Difference
In this role, you will support patient care delivery as part of the home health team, working under the direction of the Clinical Manager. You will:
  • Assist with patient care activities for a designated group of patients
  • Support the delivery of care aligned with Advocate Health at Home standards
  • Help ensure patients receive high-quality, cost-effective care
  • Promote and support patient and family rights
  • Contribute to maintaining and improving overall quality outcomes
Required Qualifications
  • Active license in the State of Illinois
  • Minimum of 2 years of medical-surgical (med/surg) experience
  • Comfortable using computer-based scheduling and documentation systems
  • Strong written and verbal communication skills
  • Effective interpersonal and teamwork skills
Compensation & Benefits
Hourly Pay Range: $24.35 - $36.55
Actual compensation is based on experience, skills, and internal equity.
