Summary:
LPN-Under the direction of the registered nurse (RN), provides nursing care in accordance with departmental and hospital policies and procedures as well as with their State Practice Act. The licensed practical nurse (LPN) assists the RN with assessing, planning, implementing, and evaluating patient care. The LPN is responsible for documentation of their care including interventions, outcomes, and patient/family education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care. The LPN is responsible for the direction and assignment of nursing interventions that may be performed by others as well as care and interventions delegated to them by the RN. Must demonstrate the knowledge and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization.
RN- The registered nurse (RN) is a professional nurse under the supervision and direction of the department manager/director. The RN works within the state defined scope of practice and adheres to department and hospital policies/procedures and regulatory requirements.
The Acute Rehab RN utilizes nursing process for assessing, planning, implementing, and evaluating patients. The care delivered is individualized and goal directed. The RN is responsible for the supervision of delegated tasks. The RN works cohesively and collaboratively with providers and other interdisciplinary health care team members to provide high quality and safe care.

Essential Job Duties and Responsibilities:
1. Demonstrates ability to appropriately modify approach and procedures to meet needs of age/diversity of population served for the following age groups:

2. Competent use of the nursing process to assist the RN in assessing priorities, formulating, implementing, evaluating and adapting the interdisciplinary plan of care to ensure quality of care is delivered.
  • Performs delegated portions of the admission assessment in accordance with specific unit standards.
  • Updates and documents patient's plan of care, once it is established by the RN.
  • Upon admission and as appropriate, assists the RN with patient/family/others discharge planning and teaching needs.
  • Delegates and assigns appropriate patient care to aides.
  • Recognizes changes in patient status based on interpretation of assessments. Reports findings appropriately and timely.

3. Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the patient in collaboration with RN.
  • Demonstrates knowledge of hospital policies and procedures for administering, verifying, and recording medications.
  • Demonstrates knowledge of commonly used meds: action, dose, and side effects.
  • Functions successfully in an emergency situation. Maintains professional composure and confidence during stressful situations.
  • Identifies patient care problems and communicates them to RN
  • Handoff reflects current plan and status of the patient.

4. Maintains timely and accurate documentation of care provided to ensure quality care and the integration of information for use by the healthcare team.
5. Performs all responsibilities/duties required by the Acute Rehab Department as defined in the scope of practice, to assure that the unique nature of the patient is addressed. This includes, but is not limited to the age of the patient served.
  • Provides appropriate patient care and initiates appropriate nursing action in a crisis situation.
  • Assists the RN in completion of the Quality Indicators Assessment Data.
  • Completes the patient team conference forms thoroughly and in a timely manner.

6. Assumes all other duties and responsibilities as necessary.
  • Demonstrates willingness to identify and/or assume activities relative to the developmental needs of the unit and the nursing department
  • Demonstrates dependability/flexibility in meeting scheduling needs of the unit and hospital.
  • Demonstrates willingness to float to other units as needed.
  • Participates in shared leadership.
  • If in charge, provides for the safety of department employees

RN-
  1. RNs assigned to direct patient care:

  • Conducts individualized patient assessment, prioritizing the data collection based on the patient's immediate condition or needs within the timeframe specified by policy.

  • Conducts ongoing assessments as determined by patient condition and policy and reprioritizes care accordingly.

  • Develops and updates the plan of care throughout the patient stay ensuring that is individualized and reflects collaboration with other health care team members.

  • Identifies patient/family discharge planning needs upon admission.

  • Performs appropriate treatments including medication administration per physician order and per policy in an accurate and timely manner.

  • Reports patient concerns and condition changes to the provider in a timely manner.

  • Performs therapeutic nursing interventions in line with the plan of care.

  • Provides individualized patient/family education customized to patient and family needs.

  • Documents all nursing cares and education accurately and timely in the electronic health record.

  • Documents response to nursing cares and education accurately and timely in the electronic health record.

  • Delegates and supervises appropriate patient care.

  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to patient and family diversity, preserving autonomy, dignity, and rights.

  • Actively participates and involves the patient and family in handoffs and bedside shift report and relates pertinent patient information.

  • Responds appropriately, professionally, with composure and confidence during emergencies and stressful situations.

  1. Performs all responsibilities/duties of an Acute Rehab RN. Department and/or Role Specific duties include:

  • Has a thorough understanding of the IRF-PAI and completes this related QI assessment in a timely manner.

  • Completes patient team conference forms thoroughly Collaborates with unit director, infection control coordinator and utilization review regarding clinical nursing issues.

  1. Assumes all other duties and responsibilities as necessary.

  • Maintains confidentiality per policy.

  • Maintains current competency by attending unit and hospital based educational offerings either in person or online.

  • Actively engages in unit activities and team meetings and attends required meetings.

  • Demonstrates active engagement in department activities and evidence-based initiatives.

  • Demonstrates dependability/flexibility in meeting scheduling needs of the department and hospital.

  • Demonstrates willingness to float to other units as needed.

  • Accepts assignment of preceptor for new staff.

  • Accepts responsibility for mentoring healthcare students.

  • Ensures the quality and safety of the patients and staff if serving in a relief charge nurse role.

Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

CERTIFICATES, LICENSES, REGISTRATIONS:

Basic Life Support (BLS) obtained within one full month from date of hire (Travelers-required upon hire) required.
Non-Violent Crisis Intervention program obtained within six full months from date of hire (Travelers-required upon hire) required.
Previous Experience Requirements:
Previous clinical experience preferred.
Skills/Knowledge Requirements:
Language Skills - Ability to read, write, speak, and understand the English language required.
