Activa Home Health is actively seeking experienced Registered Nurses for our innovative Hospital at Home Program. This opportunity is designed to attract top clinical talent for full-time and per diem roles across multiple service areas, including Vero Beach, Port St. Lucie and Stuart.

We are seeking highly skilled RNs to deliver acute-level care in the home setting, supporting patients who would otherwise require hospitalization. This role is ideal for nurses who thrive in dynamic environments, deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky), and are passionate about advancing patient-centered care models.
