Coordinates care with the interdisciplinary team, patient/family, and referring agency; the BHRN also assumes the responsibility for coordination of care.**Essential Job Functions:*** Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs* Develop, implement, and regularly update individualized plans of care to ensure quality and continuity of services* Provide skilled nursing interventions related to behavioral health and applicable medical conditions within scope of practice* Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies* Ensure patient medication profiles are accurate and current at all times* Complete admissions, resumptions of care, recertifications, and discharges in accordance with regulatory standards* Demonstrate working knowledge of OASIS and ensure documentation supports medical necessity and homebound status* Complete visit documentation accurately and within required timeframes, including same-day SNVs and OASIS within 48 hours* Utilize appropriate behavioral health and medical assessment screening tools and evidence-based practices to evaluate and support patient needs.* Monitor assigned cases to ensure compliance with payer, federal, and state requirements* Supervise and provide clinical direction to Home Health Aides as applicable* Participate in quality assurance activities, including responding to QA and coder reviews and performing clinical record audits* Maintain proficiency in EMR systems, including timely syncing of visits and device maintenance* Meet established productivity standards, including visit volume, meetings, and assigned duties* Participate in mandatory meetings, case conferences, and on-call rotation, including weekends and holidays* Adhere to organizational policies, HIPAA requirements, and professional standards of conduct* Demonstrate sound clinical judgment, accountability, adaptability, and effective communication under routine and stressful conditions**Qualifications:*** **Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.*** **Must have 1-2 years of nursing experience (within the last 3 years preferred) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric hospitalization program, or other outpatient psychiatric service.*** **Current state license as a Registered Nurse.*** **Current state Driver’s License.*** **Proof of current CPR and Hepatitis consent/declination.*** **Reliable transportation with valid and current auto liability insurance.***We are an equal opportunity employer.
