We are seeking a dynamic and relationship-driven Hospital Liaison to join our growing team.

Position Summary

The Hospital Liaison serves as the primary connection between hospitals, physicians, case managers, discharge planners, and Eastern Healthcare Group's skilled nursing facilities. This role is responsible for developing referral relationships, facilitating patient admissions, and ensuring seamless transitions from acute care settings to post-acute and long-term care services.

Key Responsibilities
  • Develop and maintain strong relationships with hospital case managers, discharge planners, social workers, physicians, and healthcare professionals.

  • Educate referral sources about Eastern Healthcare Group's services, clinical programs, and admissions process.

  • Conduct regular hospital visits to identify referral opportunities and support discharge planning efforts.

  • Review patient referrals and collaborate with clinical and admissions teams to determine appropriate placement.

  • Coordinate smooth transitions for patients from hospitals to skilled nursing facilities.

  • Maintain referral tracking and provide regular activity and performance reports.

  • Participate in networking events, healthcare forums, and community outreach initiatives.

  • Stay informed on industry trends, Medicare, Medicaid, and managed care guidelines affecting post-acute care services.

  • Meet and exceed census growth and referral development goals.

Qualifications
  • Bachelor's degree in Healthcare Administration, Marketing, Business, Nursing, Social Work, or related field preferred.
