Job Title: Registered Nurse (RN) - Case Manager
Location: 1924 Alcoa Hwy, Knoxville, TN 37920
Shift: Day Shift - 5x8 Hours (08:00 AM - 04:00 PM)
Start Date: ASAP (Contract Start: 03/09/2026)
Duration: 13 Weeks
Guaranteed Hours: Yes
Position Overview
We are seeking an experienced Registered Nurse (RN) - Case Manager with strong hospital-based case management experience. The ideal candidate will have a minimum of 5 years of hospital case management experience and proficiency in Cerner/PowerChart and discharge planning platforms.
Key Responsibilities
  • Manage a caseload of approximately 25-28 patients.
  • Perform comprehensive patient assessments and develop individualized discharge plans.
  • Coordinate care across interdisciplinary teams to ensure appropriate resource utilization and timely discharges.
  • Facilitate communication between physicians, patients, families, and post-acute care providers.
  • Complete PASRR (ASCEND) entries and ensure compliance with state and federal regulations.
  • Utilize Cerner/PowerChart and WellSky (formerly NaviHealth) for documentation and care coordination.
  • Apply PAE knowledge and ensure accurate documentation.
  • Float within scope of practice as needed to meet facility requirements.

Requirements
  • Experience: Minimum 5 years of hospital case management experience (Required)
  • EMR: Cerner/PowerChart experience (Required)
  • Experience with WellSky (formerly NaviHealth) (Required)
  • PASRR entry experience (ASCEND)
  • Knowledge of PAE processes
  • Active RN license (Pending license not accepted)
  • First-time travelers will be reviewed
  • Local candidates considered on a case-by-case basis
