Job description

Hospice Nurse Practitioner (NP)

We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) to provide face-to-face patient visits and clinical oversight throughout our South Jersey service area. This role is essential in ensuring high-quality, patient-centered hospice care while supporting compliance with regulatory requirements and collaborating closely with the interdisciplinary team.

Key Responsibilities:

  • Conduct face-to-face hospice eligibility visits in accordance with Medicare guidelines
  • Perform comprehensive patient assessments to determine continued hospice eligibility
  • Provide oversight and support to hospice teams across Bergen, Passaic, Morris, Sussex, and Essex counties
  • Collaborate with physicians, RNs, social workers, and care teams to develop and update individualized care plans
  • Ensure accurate, timely, and compliant documentation in the EMR
  • Participate in interdisciplinary team (IDT) meetings as needed
  • Provide guidance and education to staff regarding hospice regulations and best practices
  • Assist in maintaining compliance with state, federal, and accreditation standards
  • Support quality improvement initiatives and patient care outcomes

Qualifications:

  • Active and unrestricted New Jersey Nurse Practitioner license
  • Board certification as a Nurse Practitioner (FNP, AGNP, or related specialty)
  • Hospice and/or palliative care experience strongly preferred
  • Strong knowledge of Medicare hospice regulations, including face-to-face requirements
  • Excellent clinical assessment and critical thinking skills
  • Ability to work independently and manage a regional territory
  • Strong communication and interpersonal skills
  • Valid driver’s license and reliable transportation (travel required throughout North Jersey)
