About the Role:
Care Plus Hospice Care LLC is looking for a passionate and driven Hospice Marketer/Liaison to join our compassionate team in Scottsdale, AZ. In this role, you'll be the vital link between our hospice services and the healthcare community, helping connect patients and families with the dignified, high-quality end-of-life care they deserve. Responsibilities:
  • Build and maintain referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers
  • Educate healthcare professionals, patients, and families on hospice eligibility, services, and benefits
  • Conduct regular sales calls and community outreach to generate and grow referral volume
  • Collaborate with the clinical team to ensure smooth patient admissions and transitions into hospice care
  • Represent Care Plus Hospice Care at community events, health fairs, and networking opportunities
  • Track and manage referral activity, territory data, and outreach efforts using CRM tools
  • Develop and execute territory-specific marketing strategies to meet census goals
Requirements:
  • Previous experience in hospice, home health, or healthcare sales/marketing strongly preferred
  • Established relationships with referral sources in the Scottsdale/Phoenix metro area is a plus
  • Strong understanding of hospice regulations, Medicare/Medicaid eligibility, and the healthcare continuum
  • Excellent communication, presentation, and relationship-building skills
  • Self-motivated, goal-oriented, and able to work independently in a field-based role
  • Valid driver's license and reliable transportation required
  • Bachelor's degree in Marketing, Healthcare Administration, or related field preferred
