Description:
As a Hospice Aide/Certified Nursing Assistant (CNA),
you are one of the most vital components of the Interdisciplinary Team! You are the heart, the eyes, and the key communicator of your patient's needs. Your role is to follow a carefully planned out, personalized Care Plan for each patient you are assigned and to report and work closely with the RN Case Manager to ensure the care of the patient is completed and expectations
exceeded. You will effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.
Minimum Requirements:
  • Current CNA License in the State of Georgia
  • Current CPR & First Aid Certification
  • 1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
  • Utmost compassion & integrity
  • Driver's license, insurance and vehicle
  • Excellent Time Management Skills
