We're seeking Nurse Practitioners to perform in-home Annual Wellness Visits for Medicare members. These visits are conversational, consultative, and education-focused— no prescribing, no treatment plans.

Requirements

  • Active State NP license and Board Certification
  • Valid NPI
  • Minimum 3 months of wellness exam experience preferred
  • Comfort with EMR/documentation systems
  • Ability to travel within a region (visits are clustered in a 10 mile radius)
  • Bilingual/Spanish Speaking a plus
  • Give on average 2 full days per week.
