Certified Home Health Aide, Home Health

We are seeking a State of California–certified Home Health Aide to provide compassionate, in-home care to patients throughout San Diego on a per diem basis.

What You'll Do

  • Provide patient care as directed on aide care plan, reports and documents observations and care performed
  • Observe and report changes in client's condition to Supervising Clinician
  • Assist the patient in ADL's (Activities of Daily Living) as directed on aide care plan
  • Communicates patient's supply needs to Supervising Clinician or Health Care team
  • Display professional behavior at all times
  • Comply with all areas of corporate compliance program and HIPPA regulations
  • Document thoroughly and accurately in format prescribed by the agency
  • Provide companionship and patient care as directed on care plan
  • Help patients and their families understand instructions given by medically trained staff

Qualifications

  • Current certification as a Certified Home Health Aide by the State of California required
  • Must have vehicle, valid current driver's license and automobile insurance
  • Minimum 1-year experience
  • High School Graduate or equivalent
  • Current CPR card
  • Documentation to verify work experience and legal right to work in the U.S.
  • Report to work in a timely manner and adhere to call-in times for schedule changes
  • Able to provide two verifiable references
