Schedule: Friday 5:00 PM – Monday 8:30 AM


Territory: Bergen & Essex Counties, NJ

Ascend Home Health is seeking an experienced and compassionate Registered Nurse (RN) On-Call Nurse to provide skilled nursing support to our home health patients throughout Bergen and Essex Counties during weekend on-call hours.

The RN On-Call Nurse is responsible for responding to patient and caregiver needs during assigned on-call hours, completing skilled nursing home visits when necessary, assessing changes in patient condition, and coordinating care with physicians and other members of the healthcare team.

Responsibilities
  • Assume responsibility for assigned patient visits and the on-call schedule, including the assessment, planning, implementation, and evaluation of nursing care.

  • Respond to patient and caregiver calls during assigned on-call hours and determine the appropriate clinical intervention.

  • Complete skilled nursing home visits as needed throughout Bergen and Essex Counties.

  • Assess patients' physical and clinical status and identify changes requiring intervention or escalation.

  • Complete patient assessments to determine ongoing home health and skilled nursing needs.

  • Regularly re-evaluate patients' nursing needs and use assessment findings to determine appropriate care plan interventions.

  • Provide skilled nursing care in accordance with physician orders and the patient's individualized plan of care.

  • Perform skilled nursing procedures and treatments within the RN's scope of practice.

  • Administer medications and treatments in accordance with physician orders, applicable federal and state regulations, and Ascend Home Health policies.

  • Communicate changes in patient condition to the physician, clinical supervisor, and other appropriate members of the care team.

  • Coordinate care with physicians, therapists, home health aides, and other healthcare professionals as appropriate.

  • Provide patient and caregiver education regarding medications, treatments, disease management, safety, and the established plan of care.

  • Maintain accurate, timely, and complete clinical documentation at the point of service in accordance with agency policies and applicable Medicare and regulatory requirements.

  • Maintain regular communication with the direct supervisor regarding assignments, patient concerns, changes in condition, and other clinical matters.

  • Identify patient needs for additional services, medical equipment, supplies, or other resources and coordinate appropriately.

  • Participate in on-call responsibilities in accordance with Ascend Home Health policies and procedures.

  • Maintain patient confidentiality and comply with HIPAA and all applicable regulatory requirements.

  • Travel locally throughout Bergen and Essex Counties to provide patient care as needed during scheduled on-call hours.

Education
  • Associate Degree in Nursing required.

Professional Experience & Requirements
  • Current and unrestricted New Jersey Registered Nurse (RN) license required.

  • Previous home health experience required.

  • Previous on-call or field nursing experience preferred.

  • Excellent clinical assessment and critical-thinking skills.

  • Excellent documentation skills and the ability to work independently.

  • Strong communication and interpersonal skills.

  • Valid driver's license and reliable transportation required.

  • Must be available to travel throughout Bergen and Essex Counties.

  • Must be available for the assigned Friday 5:00 PM – Monday 8:30 AM on-call schedule.
