  • Provide skilled nursing care to patients in their homes throughout Southwest Tucson
  • Conduct comprehensive patient assessments and develop individualized plans of care
  • Administer medications, treatments, injections, and wound care according to physician orders
  • Monitor patient progress and communicate changes in condition to physicians and the care team
  • Educate patients and families on medications, disease management, safety, and wellness
  • Coordinate care with physicians, therapists, social workers, and other members of the interdisciplinary team
  • Supervise and document care provided by LPNs and Home Health Aides (HHAs), as appropriate
  • Maintain timely, accurate, and compliant clinical documentation
  • Participate in interdisciplinary care planning to help patients achieve the best possible outcomes at home

Qualifications
  • Current Arizona RN license in good standing
  • Minimum of one year of RN experience preferred
  • Home health, hospice, rehabilitation, med-surg, or critical care experience is a plus
  • New Graduate RNs are encouraged to apply through our Ambassador Program
  • Current CPR certification
  • Valid driver's license and reliable transportation
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication, organizational, and interpersonal skills
  • Ability to independently manage a full-time patient caseload
