Job description

Enjoy the flexibility home care can provide! Pinnacle is seeking an experienced, dedicated, and caring full-time Registered Nurse (RN) to join our home health team.

The RN is responsible for planning, coordinating, providing, and documenting care for home health patients from day of admission through day of discharge. Utilizing physicians’ orders and your professional skills, you will be collaborating with the patients' interdisciplinary team, and case manage care by developing and implementing a plan of care that meets each patient’s specific needs. This position reports directly to the Director of Nursing.

This is a full-time position in the field for the following area: Baton Rouge.
