Licensed Practical Nurse (LPN) - Home Care
Overview
Join our dedicated care team as a Licensed Practical Nurse (LPN) specializing in Home Care. In this role, you will play a vital part in supporting our clients' health and well-being within the comfort of their own homes. You will conduct in-home visits to monitor client status, provide essential nursing support, and ensure that our non-medical care plans are being followed effectively. You will be a key point of contact for clients, families, and our caregiving staff, helping to maintain quality of life and safety for those we serve in and surrounding the Atlanta area.
Duties
• Clinical Monitoring: Conducts routine in-home nursing visits to assess the client's physical and emotional condition, documenting findings in accordance with Georgia state regulations for home care.
• Plan of Care Implementation: Implements and supports the established plan of care, providing guidance to caregivers on proper execution.
• Documentation: Maintains accurate, timely documentation of all client assessments, nursing notes, and observations regarding changes in condition.
• Communication: Reports significant changes in a client's status or health to the Care Coordinator, and RN as required.
• Education: Provides education and training to caregivers and family members regarding the client's health needs, safety precautions, and activities of daily living.
• Care Coordination: Collaborates with the care team to ensure seamless service delivery and assists in evaluating the effectiveness of the support provided.
• Compliance: Adheres strictly to Georgia Nurse Practice Act regulations for LPN scope of practice and company policies/procedures.
• Quality Assurance: Participates in clinical audits and quality improvement activities to ensure the highest standards of care.
• Professional Development: Attends required staff meetings.
• Team Support: Assists with caregiver skills trainings and supports the onboarding process for new field staff as needed.
Compensation & Requirements
• Compensation: $45.00 - $50.00 per visit.
• Travel: 75% local travel required within the Atlanta, GA area to provide in-home care services.
• License/Certification: * Current, unencumbered LPN License in the State of Georgia (Required).
• Valid Driver's License (Required).
• Proof of Current Car Insurance (Required).
• Shift Availability: Day Shift (Required).
