Job description

Home Health Physical Therapist

Full-time home health physical therapist providing patient-centered care in patients' homes across Springfield, Missouri. Works Monday-Friday with a supportive team and flexible autonomy.

Key Responsibilities:
  • Perform comprehensive patient evaluations and develop individualized treatment plans.
  • Conduct start-of-care assessments and complete OASIS documentation accurately.
  • Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence.
  • Complete patient recertifications and reassessments as required.
  • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety.
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams.
  • Monitor patient progress and modify treatment plans to maximize outcomes.
  • Maintain timely and accurate clinical documentation using Matrix EMR.
  • Deliver compassionate, patient-centered care in a home-based setting.
Qualifications:
  • Doctorate or Master's degree in Physical Therapy from an accredited program
  • Active Missouri Physical Therapist license
  • Strong communication, organization, and time management skills
  • Ability to work independently and manage a daily patient schedule
Preferred Skills:
  • Current CPR/BLS certification
  • Experience with OASIS documentation and Matrix EMR
