Physical Therapist (PT) - Home Health | McDonough, GA | PRN | $72-$85 Per Visit
Stern at Home Therapy is hiring a PRN Physical Therapist (PT) to provide one-on-one care to patients in their homes throughout McDonough, GA, and surrounding communities.
This opportunity is ideal for a Physical Therapist looking to supplement an existing schedule with flexible home health visits while maintaining control over when and how much they work.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes.
You'll have the clinical autonomy to develop individualized plans of care while our administrative team supports you with scheduling, referrals, authorizations, and patient coordination.
Responsibilities Include:
  • Perform physical therapy evaluations and develop individualized plans of care.
  • Provide one-on-one physical therapy treatments in patients' homes.
  • Help patients improve strength, balance, mobility, endurance, and functional independence.
  • Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
  • Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
  • Complete accurate and timely clinical documentation.
  • Communicate with physicians, caregivers, families, and the interdisciplinary care team.
Compensation:
PRN - Minimum 5 Patients:
  • $72 per follow-up visit.
  • $85 per evaluation.
PRN compensation is paid at a fixed per-visit rate with no tiered volume model.
