Effective Date: 1/1/2025

Reports To: Clinical Director

Total Care Connections is expanding into skilled care, and we are seeking an experienced and mission driven Registered Nurse Case Manager (RNCM) to join our growing team. This is a key opportunity to shape our pediatric and geriatric private duty nursing programs as we elevate our services across Arizona.

If you are passionate about providing exceptional home based nursing care, thrive in a leadership role, and want to be part of an organization driven by servant leadership, radical generosity, and a commitment to the extraordinary, we want to meet you.

About the Role

The RN Case Manager oversees and coordinates high quality private duty nursing services for pediatric and adult clients. You will guide field nurses, support families, ensure regulatory compliance, and promote a culture of excellence as TCC expands its skilled care division.

This role blends hands on clinical oversight with meaningful client connection while providing you the chance to help build something extraordinary.

Key Responsibilities

Clinical Leadership

  • Supervise and support RNs, LPNs, and CNAs to ensure care aligns with individualized care plans.
  • Conduct routine case reviews, field visits, and provide ongoing clinical coaching.
  • Assist in onboarding, training, and professional development of clinical staff.

Care Coordination

  • Oversee assessment, planning, implementation, and evaluation of care plans for both pediatric and adult private duty nursing clients.
  • Act as a liaison between families, physicians, specialists, and interdisciplinary partners to ensure continuity of care.
  • Participate in patient admissions, including comprehensive assessments, recertifications, transfers, resumptions, and discharges.

Quality Assurance and Compliance

  • Ensure staffing, documentation, and care delivery meet state, federal, and accreditation standards.
  • Conduct periodic audits of clinical notes and home visits to evaluate care quality.
  • Stay up to date with regulations affecting home health and PDN services.

Risk Management

  • Identify and address potential care risks including infection control, medication safety, and emergency procedures.
  • Provide guidance on critical incidents, patient complaints, and staff concerns, escalating issues as needed.

Documentation and Reporting

  • Ensure accurate, timely charting of all patient care activities.
  • Submit required clinical reports to leadership and regulatory bodies.

Resource Allocation

  • Assist in managing medical supplies, equipment, and resources to maintain efficient operations.

Client and Family Engagement

  • Educate families on disease management, care expectations, and available resources.
  • Advocate for client rights, safety, and preferences in all aspects of care.

Qualifications

  • Current and unrestricted RN license in Arizona.
  • BSN preferred; ADN required.

Experience

  • At least 1 year of RN case management experience within a private duty nursing (PDN) organization is required.
  • Must have direct case management experience serving both pediatric and adult/geriatric PDN clients.
  • Experience with PDN admissions, care planning, authorizations, staffing coordination, and family communication is essential.
  • Knowledge of home health or skilled nursing regulations including Medicare and Medicaid preferred.

Skills

  • Strong leadership and team management abilities.
  • Excellent communication and interpersonal skills.
  • Proficiency with electronic health records and care documentation.
  • Strong clinical judgment and problem solving ability.

Certifications

  • CPR and First Aid required.
  • Specialty certification in home health or related fields preferred.

Working Conditions

  • Primarily office based with regular travel to client homes for supervision and audits.
  • May involve lifting, bending, or assisting with mobility.
  • Flexible schedule with potential on call responsibilities.
