Job description

Summit Home Care and Hospice is searching for an experienced and motivated Speech Therapist/Speech Language Pathologistย to join our team. Weโ€™re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.

Speech Therapist/Speech Language Pathologist:

  • Part-Time:ย Monday - Friday; 20 Hours/Week (Flexible Scheduling)
  • Coverage Location:ย Washington, IN & Surrounding Areas (Including Daviess, Martin, Knox & Duboisย Counties)
  • Travel: Required + Milage Reimbursement
  • Work Setting:ย On the Road for Patient Visits & Training at the Local Office or Remotely
  • Compensation: Pay per Visit (Rates Vary Depending on Service, Experience & Coverage Area) ย 

Responsibilities:

  • The Speech Therapist/Speech-Language Pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.ย 
  • Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve patient's function.
  • Administers speech therapy program utilizing specialized therapeutic techniques and/or equipment.
  • Recommends communication devices/aids as indicated.
  • Works with referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits notes per organization policy.
  • Maintains clinical competency in speech-language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instruct patient, family/caregiver and other Organization health care personnel in the patient's treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech-language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech-language pathology services.

Qualifications:

  • Possesses a master's or doctoral degree in speech-language pathology (SLP).
  • Must meet either of the following requirements: be licensed as a speech-language pathologist by the State in which the individual furnishes such service; or if the State does not license speech-language pathologists: must have successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience); performed not less than 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field; and successfully completed an approved national examination in SLP.
  • A minimum of one (1) year's clinical experience in Speech-Language Pathology is preferred.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements
  • Recent TB test required.
  • Demonstrates good verbal and written communication, and organization skills.
