As aย Home Healthย Physical Therapist,ย hereโ€™sย a big-picture view of whatย youโ€™llย do:ย 

  • Provide home-basedย physical therapy toย patientsย who have lost mobility, strength and functionality due to an illness or injuryย ย 
  • Workย as part ofย aย home health teamย which mayย include anย RN, LPN, CNA, HHA,ย OTย andย SLP, focused on the patientโ€™s plan of careย and goalsย ย ย 
  • Observeย movements,ย conduct assessments,ย documentย progressย andย modifyย care plan as neededย 
  • Assistย patient withย exercises and hands-on therapyย to manage pain and increase mobilityย 
  • Educate patientsย on their plan of care,ย establishedย goals, exercises and self-careย 
  • Coach family onย theย plan of care, therapy goals and safelyย assistingย with exercisesย 

A few must-havesย for Home Healthย Physical Therapists:ย 

  • Graduate ofย anย accreditedย Physicalย Therapistย Programย and activeย PTย license in Michiganย (NEW GRADS WELCOME!)
  • 1 year of clinical experience, preferably within home health in the last 5 years
  • CPR certificationย 
  • Good clinical judgement, strong interpersonal skills,ย dexterityย and compassionย
