We’re a locally owned, state-licensed home care agency proudly serving the Minneapolis–St. Paul metro area with trusted, high-quality private duty nursing and home care services. We’re seeking an experienced and compassionate Per Diem Registered Nurse (RN) to complete client assessments, care plans, and clinical oversight visits for our private-pay home care clients. This flexible, independent position is ideal for an RN looking to supplement hospital, hospice, or clinic work — or to enjoy meaningful field-based nursing with work–life balance.

Position Summary:

Offering $100/visit. The Per Diem RN Assessment Nurse provides skilled, person-centered nursing assessments, develops individualized care plans, and ensures compliance with Minnesota Department of Health (MDH) home care regulations. The RN also supports field caregivers, performs supervisory visits, and promotes safe, quality care in the client’s home setting. This role is perfect for nurses with strong clinical judgment, home care experience, and a passion for working independently in the community. Key Responsibilities

  • Conduct initial, comprehensive nursing assessments and periodic reassessments in clients’ homes.
  • Develop, implement, and update individualized service plans (ISPs) per MN home care rules.
  • Provide clinical oversight for Caregiver, CNA, and HHA staff; perform supervisory visits as required.
  • Ensure all documentation meets state licensing and agency quality standards.
  • Communicate effectively with clients, families, caregivers, and interdisciplinary team members.
  • Participate in quality assurance, compliance, and clinical training initiatives.
  • Maintain professional ethics, confidentiality, and high standards of nursing practice.

Requirements:

QualificationsRequired:

  • Active Registered Nurse (RN) license in Minnesota
  • Minimum 3 years of nursing experience (home care, hospice, or community health preferred)
  • Knowledge of MN home care licensing rules and client assessment procedures
  • Excellent assessment, clinical documentation, and communication skills
  • Reliable transportation and valid driver’s license
  • Self-motivated and organized with strong time management skills

Preferred:

  • Experience as a Home Health RN, Case Manager, or Field/Assessment Nurse
  • Prior experience with Alzheimer’s, dementia, or chronic illness care
  • Familiarity with electronic health record (EHR) systems
