Job description

Are you a Registered Nurse looking to elevate your career beyond the hospital walls?

Step into a role where your clinical expertise drives profound change and your compassion builds life-altering relationships. Support Broker Services, LLC invites experienced RNs to pioneer a new path in personalized home healthโ€”delivering elite, one-on-one care to a participant in Wisconsinโ€™s innovative IRIS (Include, Respect, I Self-Direct) program.

This is more than a nursing positionโ€”itโ€™s a professional venture where you become the cornerstone of someoneโ€™s world.

Why This Role Is Different:
  • Autonomy & Impact: Exercise full scope of practice in an environment where your decisions directly shape patient safety, comfort, and quality of life.
  • Clinical Excellence: Specialize in complex seizure management and behavioral support for individuals with Lennox-Gastaut Syndrome (LGS) and treatment-resistant epilepsy.
  • Meaningful Connection: Forge a trusted partnership with your patient and their family in the comfort of a home settingโ€”away from the rushed pace of institutional care.
  • Schedule Flexibility: Design your work-life balance with 12-hour shift optionsโ€”day (7 AM โ€“ 7 PM) or overnight (7 PM โ€“ 7 AM).

Who We Are:
Support Broker Services is redefining home-based care through highly individualized, participant-directed support. We collaborate with Forward Health to empower those with complex medical needs to live fully and independently.

What Youโ€™ll Do:
  • Provide skilled nursing care, including advanced seizure response and proactive behavioral interventions.
  • Develop and implement personalized care plans in collaboration with a dedicated multidisciplinary team.
  • Educate and empower family members and support staff in best-practice care techniques.
  • Serve as a clinical advocate and expert within the clientโ€™s home, ensuring dignity and respect in every interaction.

Who Weโ€™re Looking For:
  • An RN licensed in Wisconsin, driven by purpose and clinical curiosity.
  • A nurse with experience in neurology, epilepsy management, behavioral health, or complex home care.
  • A compassionate critical thinker who thrives in autonomous, dynamic environments.
  • Someone who meets all WI DHS caregiver requirements, including background checks and registry reviews.

What We Offer:
  • Competitive Compensation: $25.00 โ€“ $35.75/hour, reflective of your experience and specialization.
  • Professional Growth: Become a leader in home-based complex careโ€”a growing and vital field.
  • Supportive Collaboration: Join a team that values clinical excellence, innovation, and humanity.
  • Fulfillment: See the tangible impact of your care every day.

If youโ€™re ready to use your nursing skills in a deeply human wayโ€”and redefine what it means to make a differenceโ€”we invite you to apply.

Final hiring decision is made by the participant/guardian.

Join us. Care with purpose.
