Home Based Primary Care Case Management Registered Nurse (Rn)

The Home Based Primary Care (HBPC) Case Management Registered Nurse (RN) is responsible for providing competent, evidence-based care and for providing case management, assessment, triage, veteran education, resource coordination, medication & treatment coordination, and care planning. This nurse plans & delivers technically competent care in the home of veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, & palliative care.
