Job description

Make a Difference in a Home-Based Setting

Ability Beyond Barriers is seeking a compassionate Licensed Practical Nurse (LPN) to support adults with intellectual and developmental disabilities in community-based residential homes.

This is NOT a nursing home or hospital role.
You will provide nursing services in small group homes where individuals live, work, and participate in the community.

What You’ll Do

As an LPN in Residential Services, you’ll integrate healthcare into everyday life—helping individuals live safely, independently, and with dignity.

Key Responsibilities
  • Provide person-centered nursing care in home-like residential settings
  • Administer medications, monitor health status, and complete treatments
  • Maintain accurate MARs and clinical documentation
  • Coordinate medical appointments and follow-up care
  • Train and support Direct Support Professionals (DSPs) on health-related tasks
  • Respond to changes in condition and communicate with the care team
  • Promote infection control, safety, and wellness in the home environment

What Makes This Role Unique
  • Work in small residential group homes (not institutional settings)
  • Build meaningful, long-term relationships with individuals you support
  • Focus on quality of life—not just clinical care
  • Be part of an interdisciplinary team that includes DSPs, coordinators, and families
  • Combine nursing skills with advocacy and community inclusion

Requirements
  • Active South Carolina LPN license (required)
  • Graduate of an accredited Practical Nursing program
  • Valid driver’s license (travel between homes required)
  • Experience with I/DD or community-based care is a plus

Work Environment & Schedule
  • Work conducted in multiple residential homes across the community
  • May include evenings, weekends, or on-call rotation as needed
  • This is an Essential Employee role (must report during emergencies)

Pay & Exceptional Benefits

We value our nurses and offer a strong compensation and benefits package:

  • $26.50 per hour
  • Health, Dental, and Vision Insurance through the
South Carolina Public Employee Benefit Authority (PEBA)
  • Multiple Life Insurance options
  • Participation in the South Carolina State Retirement System (SCRS)
  • 13 Paid Holidays
  • Annual Leave and Sick Leave

Benefit Eligibility Timeline

  • Insurance benefits begin on the 1st day of the month following full-time hire
  • Annual Leave, Sick Leave, and Holiday Pay begin after the 31st day of full-time employment

Why Ability Beyond Barriers?
  • Mission-driven work with lasting impact
  • Stable, state-supported benefits
  • Respect for your clinical judgment and professional role
  • A collaborative, supportive residential services team
  • The opportunity to practice nursing in a relationship-based, community-focused environment
  • Starting Bonus $1,200.00 - $600.00 at completion of 45-Days; $600.00 at completion of 90-Days

Ready to Apply?

If you’re looking for a nursing role where compassion, advocacy, and clinical skill come together, we encourage you to apply and grow with Ability Beyond Barriers.

Ability Beyond Barriers is an Equal Opportunity Employer.
