Description
AuthoraCare Collective is currently seeking a Nurse Practitioner (Home Based Primary Care). This is a full-time position; Monday - Friday 8:00am - 5:00pm (office base will be High Point) with on-call responsibility.
CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
The Nurse Practitioner is responsible for delivering comprehensive primary care, palliative care, and chronic disease management to patients in their homes. Conducts physical examinations, diagnosing and treating illnesses, managing chronic conditions, engaging in advanced care planning discussions, and coordinating care with other healthcare providers. Focuses on health promotion, disease prevention, and patient education to ensure continuous and holistic care for home-bound patients. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.
Responsible for building and maintaining therapeutic relationships with patients and caregivers, including providing emotional support and education, is a key aspect of this role. Collaborates closely with the interdisciplinary team and effectively communicate, both verbally and in writing, with team members, patients, caregivers, physicians, pharmacists, and other healthcare professionals and community service organizations. Participation in on-call coverage for the Integrated Health Services population is required as assigned.

Requirements
Education and Experience:
  • Graduate of an accredited School of Nursing
  • Current Nurse Practitioner license in North Carolina.
  • Minimum of two years of experience in an acute care, home health, long-term care, or outpatient/office setting required.
  • CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
  • Palliative care experience preferred.
  • Excellent assessment and clinical skills, with strong interpersonal and communication abilities.
  • Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
