Description

Job Summary: The Registered Nurse is responsible for providing safe and effective delivery of patient care through assessment, planning, implementation and evaluation of patient and family needs in the home care environment. Directs and supervises the home health aide in providing quality patient care services. The RN is responsible for organizing and managing cases including coordination with other professional disciplines (PT-OT-ST-MSW).

Job Responsibilities:

  • Supports patient's rights and responsibilities, including respect of cultural and religious diversity.
  • Demonstrates flexibility to meet staffing needs of department/organization appearance.
  • Meets productivity standards, accepting low census time and/or extra time as needed.
  • Demonstrates leadership abilities while appropriately following the chain if command.
  • Performs and evaluates appropriate and complete evaluation based on patient diagnoses and specific to care, leading to appropriate care planning decisions.
  • Effectively utilizes nursing process to ensure optimal nursing care and patient outcomes.
  • Provides age –appropriate and population specific patient care in a safe, skillful, accurate and efficient manner to primarily adult and geriatric populations as indicated.
  • Assesses patient and family learning needs and provides ongoing education and evaluation.
  • Adheres to standard precautions and infection control standards.
  • Documents readiness to learn, teaching, barriers and outcomes.
  • Reassesses significant change in patient's condition and updates care plan appropriately.
  • Identifies patient care needs to formulate and/or contribute to the discharge plan.
  • Completes documentation accurately and in a timely manner, according to policies and procedures.
Qualifications

Education Requirements and Other Requirements:

Education Level: Graduate of an accredited school of nursing. Certification/Licensure: Licensed in the state of Illinois. BLS certification required. Experience Requirements: Two years of experience in a health care setting is required. Home health experience is preferred. Other Requirements: Driving and reliable transportation is a necessity.
