Position Summary

The Clinical Pharmacist provides comprehensive pharmaceutical care through medication therapy management, clinical consultation, medication order review, patient monitoring, and interdisciplinary collaboration. This role serves as a key medication resource for medical staff, nursing staff, pharmacy colleagues, patients, and caregivers while supporting quality improvement, education, and patient safety initiatives.

Key ResponsibilitiesClinical Pharmacy Practice
  • Review patient records and medication orders to ensure safe, appropriate, and effective drug therapy.
  • Evaluate patient conditions and identify opportunities to optimize medication treatment plans.
  • Monitor medication therapies and intervene when necessary to improve outcomes and reduce risk.
  • Assess treatment effectiveness and recommend therapeutic adjustments.
  • Provide medication consultation to physicians, nurses, and patients.
  • Advise on medication dosing, administration, contraindications, and potential drug interactions.
  • Practice Medication Therapy Services through comprehensive patient assessment and evidence-based recommendations.
  • Collaborate with providers to clarify orders and support optimal treatment decisions.
Patient Safety & Quality
  • Identify and resolve potential clinical and operational medication-related issues.
  • Participate in drug utilization evaluations and adverse drug reaction reporting.
  • Support quality assurance initiatives and recommend process improvements.
  • Maintain compliance with regulatory, licensing, and accreditation standards.
  • Follow all confidentiality, safety, and risk management policies and procedures.
Education & Leadership
  • Serve as a drug information resource for pharmacy, nursing, and medical staff.
  • Present educational programs, in-services, and lectures to healthcare professionals.
  • Assist with competency development and staff education initiatives.
  • Precept and mentor Pharmacy students, residents, and learners.
  • Maintain current knowledge of pharmacy practice standards and clinical advancements.
Collaboration & Operations
  • Coordinate closely with pharmacy and hospital operational teams.
  • Participate in departmental initiatives, policy reviews, and medication-use evaluations.
  • Serve on interdisciplinary committees as requested.
  • Build and maintain strong professional relationships across the healthcare team.
  • Communicate effectively during shift transitions to ensure continuity of care.
QualificationsEducation
  • Bachelor of Science (BS) in Pharmacy or
  • Doctor of Pharmacy (PharmD) from an accredited School of Pharmacy
Licensure
  • Current Pharmacist License in the state of practice

  • Hospital or clinical pharmacy experience preferred.
