Job Description:
This position is for a Home Health RN responsible for managing and providing home-based care. The role requires experience with Home Care Home Base and Home Health, and entails no clinical involvement but direct case management of adult patients. Work is scheduled from Monday to Friday with weekend rotations, requiring a current license in hand upon submission.
Responsibilities:
  • Case manage roughly 30 adult patients using a point system of 6.5 points a day.
  • Perform start of care, recertifications, and daily visits as scheduled.
  • Coordinate with clinical manager or educator for support.
Requirements:
  • Must have Home Care Home Base and Home Health experience.
  • Licensed RN in the state of Kentucky, in hand upon submitting.
  • Minimum of 1 year of experience on a Medsurg floor in a hospital setting.
  • BLS certification from AHA required.
